The ICCA form refers to the Individual Child Care Agreement, a form that is used each time a foster youth changes placements. Ohio foster care youth have expressed repeated concerns about this form, and asked for it to be amended.

The latest information that we have is that there is no current version of the ICCA in a pdf form. Apparently when it went online and into SACWIS, the many new drop down options for caseworkers to click makes it difficult to create a corresponding pdf.
